The welfare assessment is the ejection record
Why an ejection form should refuse to complete without a welfare assessment, and what that assessment needs to contain.

If a welfare section can be skipped, it will be skipped, and it will be skipped exactly on the night it mattered.
Make it a gate, not a field
In Vantra an ejection or refusal cannot be completed until the welfare assessment is done. Not a warning, not a red asterisk — the record does not close. That is an uncomfortable design decision that takes about twenty extra seconds and removes an entire category of later problem.
Review, automatically
Force used, or a safeguarding flag, holds the record for supervisor review. Nobody has to remember to escalate at four in the morning, because the process does it.
Photographs need a policy, not a habit
Photographing a member of the public is a data-protection decision. It should be governed by an explicit per-event policy, need a written justification, and have its own retention period. A phone camera roll is none of those things.
